In your order posted, OH, ND, and MT is my guess. But it is tough to guess.
Comment on Flickr #4 Lake Erie Break. In ice venacular, the break in your photo is an example of a "lead" (long crack showing open water). Other interesting features in the photo is a lot of rafted ice, where one sheet slides over the top of another. The rafted ice is thicker and therefore lighter in color.
